How to Report a possible Side Effect (Adverse Event Reporting):
In order to process your information quickly and effectively, please endeavor to provide as much information as possible. At the minimum, include patient details (such as initials, gender, age, etc), contact information of the reporter, description of the side effect/adverse event (such as signs and symptoms, date event started, date ended, etc), and details of the suspected drug.
